Testicular biopsy

At IVF OCHOA we perform testicular biopsies when it is necessary to obtain sperm directly from the testicle. This is an outpatient surgical procedure which allows for Intracytoplasmic Sperm Injection (ICSI) as part of an IVF treatment.

Testicular biopsies are a necessary procedure when a man’s semen contains no sperm (azoospermia). The root causes for azoospermia differ, but may include an obstruction of the seminal duct, a previous vasectomy, or congenital alterations in the reproductive structures. Testicular biopsies can also be conducted when there exist abnormally low levels of sperm or when these are of poor quality.

There are different sperm extraction procedures:

1. TESE

Testicular sperm extraction

This procedure may be advised when a man has no mature sperm in the epididymis. The sperm sample is obtained from a small incision in the scrotum and under local anaesthesia. TESE is advised when the epididymis is obstructed as a result of a previous surgery or infection, problems related to the overall structure of the reproductive organs, or an obstruction of the seminal vesicle ducts.

2. TESA

Testicular sperm aspiration

Instead of an open biopsy, TESA involves aspiration with direct negative pressure on the testicular tissue, via a small needle attached to a syringe. Although the technique is minimally invasive, it results in fewer sperm being recovered.

At IVF OCHOA we regularly perform the surgical biopsy (TESE). The obtained testicular tissue sample is processed in our laboratory by an embryologist in order to recover any spermatozoa that may be present.

Once the spermatozoa are extracted, they can be used directly in an IVF-ICSI cycle or cryopreserved for later use. Each sample in the sperm bank is divided into several doses. Once the existence of spermatozoa has been confirmed and a sample been cryopreserved, ovarian stimulation may commence for the female partner.
